On this page you will find documents and resources needed to complete your venue rental at the Scripps Seaside Forum.

Please complete the following two documents and return to your venue manager via email.

Credit Card Authorization Form - please do not send this via email for confidentiality purposes. You can download, print and mail in this form to us directly, or you may call your venue manager directly to make the final venue payment and parking payment.

Parking Information and Instructions please download, read and coordinate parking arrangements with your venue manager directly.

Liability Insurance Requirements instructions - please download and read this one sheet before applying for your day of event insurance. We have specific requirements needed. Please email this document to your vendors directly as well.
